For Employees

Your Dental Benefit, Your Dentist

Dollar Based Dental is a self-funded dental plan offered through your employer. It gives you the freedom to choose the dentist you want, a benefit structure that's easier to understand, and care decisions that stay between you and your provider.

What You Get

What Dollar Based Dental Means for You

1

Choose Your Own Dentist

You are not restricted to a narrow dental network. You can see the dentist you already know and trust, or choose any licensed dental provider, subject to your employer's plan documents.

2

Understand Your Benefit

Dollar Based Dental uses a dollar-based benefit structure. Your plan documents clearly define what is covered and how much — no confusing procedure code lookups or UCR fee schedule surprises.

3

No Network Frustration

Traditional dental plans often frustrate employees when their preferred dentist leaves the network. Dollar Based Dental removes that dependency entirely.

4

Your Care Stays Between You and Your Dentist

Treatment decisions are made between you and your provider — not dictated by a carrier's network contract. Your dentist can recommend the care that's right for you.

Common Questions

Employee FAQ

Can I use my current dentist?

In most cases, yes. Dollar Based Dental is designed as a provider-choice plan. You are not restricted to a network. Check your employer's plan documents for any specific requirements.

How do I know what my benefit covers?

Your employer's plan documents define your covered services, annual maximum, deductible, and any exclusions. Contact your HR department or Eagles for a copy of your plan documents.

How do I submit a claim?

Claims are submitted to Eagles, the plan's third-party administrator. Your provider can submit on your behalf, or you can submit after receiving a statement. Your plan documents include claim submission instructions.

What if my dentist has never heard of Dollar Based Dental?

That's okay. Dollar Based Dental does not require providers to sign a network agreement. You can show your ID card and explain that your plan is a self-funded employer benefit administered by Eagles. Your provider can submit a standard ADA dental claim form.

Is there a deductible?

Deductibles depend on your employer's plan design. Review your plan documents or contact your HR department for your specific plan terms.

What is the annual maximum?

Annual maximums vary by plan design. Your employer selects the plan structure. Common plan designs range from $1,500 to $10,000 per person per year. Check your plan documents for your specific maximum.
