Flexible Plans Built Around Your Workforce
Dollar Based Dental plans are fully customizable by employer. The three sample designs below illustrate how employers typically structure coverage — from essential preventive benefits to comprehensive major and implant coverage. Every plan can be tailored to fit your group's needs and budget.
Dollar-Based Benefits
Benefits are defined as dollar amounts — not percentages of a carrier's fee schedule. The employer sets the annual maximum and coinsurance rate. Members know exactly what their plan covers.
No Network Required
Members choose any licensed dental provider. There is no restricted network, no in-network vs. out-of-network distinction, and no geographic coverage gaps.
Fully Customizable
The plan designs shown here are samples. Employers can adjust annual maximums, deductibles, coinsurance rates, and covered services to match their workforce and budget.
